愈合:癥狀消失,BMD(Bone mineral density,骨密度檢測)或BMC(bone mineral content,骨礦含量)在正常成人骨密度平均值的1個標準差(SD)之內;好轉:骨質在原有基礎上增加,即BMD或BMC較診斷治療時基礎上平均增加1個標準差以上;控制:BMD或BMC的測定值較診斷治療時未有明顯改變,癥狀消失或得以控制;無效:癥狀無改善,BMD或BMC值減少明顯,甚至出現骨折。

Abstract Osteopenia and osteoporosis is an increasing comorbidity in patients with HIV/AIDS. This review introduces epidemiology of osteopenia/osteoporosis in this population, discusses possible risk factors and corresponding mechanisms, elucidates strategies in screening, diagnosis and monitoring, as well as presents nonpharmacologic and pharmacologic therapy.

Comparative analysis of PFNA and DHS in the treatment of intertrochanteric fracture

[Abstract] Objective To compare the DHS and the PFNA in the treatment of intertrochanteric fracture, and to explore reasonable treatment of intertrochanteric fracture internal fixation treatment. Methods 58 cases of treatment of intertrochanteric fracture in our hospital from January 2013 to December 2013 were retrospective analyzed, including 31 cases in PFNA group and 27cases in DHS group. The operation time, intraoperative blood loss, hospitalization days and postoperative recovery of hip function index were analyzed. Results 58 patients were followed up for 7 to 15 months, an average of 11 months. Compared with the DHS group, operation time, intraoperative bleeding, less complications, according to Harris scoring criteria, the hip joint function recovery of PFNA group were shorter. Conclusion The trauma PFNA treatment in the treatment of intertrochanteric fracture is small, and it has the advantage of reliable fixation, less complications and good functional recovery. It is the first choice for the treatment of intertrochanteric fracture.

DHS又稱 Richard 釘,屬于偏心固定,其固定于股骨的外側皮質,主要通過較長的力臂及螺釘發揮強有力的內固定作用。DHS具有雙重加壓的作用,能為骨折部位提供強有力的內固定支撐作用,縮短骨折愈合時間,且其對于髓內的破壞相對有限,特別是股骨干部分髓內的影響較小,為骨折的愈合提供了相對較為有利的骨內部環境。因此在臨床上,DHS曾被認為是治療股骨粗隆間骨折的主要方式,而且目前仍廣泛應用于股骨粗隆間骨折的治療。
